Does the worksheet handle different utilisation by membership category?

Not in this simple version, which applies average utilisation to the whole covered population. For different rates by category or age group, copy the Services sheet for each group.

Should I use tariffs or unit costs?

Tariffs show what the scheme will pay; unit costs show what services cost to provide. Record which you used for each service; the difference matters for provider sustainability.
